The information you provide in this request form will be reviewed and confirmed prior to being issued a Permit. Hualalai Resort requires this information for resort security, to prevent unauthorized resort entry, and accurately grant permission to worksites on Hualalai Resort grounds.

IDENTITY PROTECTION / DRIVER LICENSE PHOTOS

ENTRY / ACCESS

Authorized drivers will be asked to present their driver's license (current & unexpired) at the security gate prior to entry. (State Issued - U.S. Driver's License only. NO international driver’s licenses accepted.)

SIGNATURE AUTHORIZATION FOR PERMITS

Drivers or Pick Up Persons will be asked to provide identification for confirmation at time of picking up a pass. Unauthorized requests will not be processed. Supervisors may be contacted to confirm authorization.

DRIVER & VEHICLE INFO

ALL vehicles and drivers must be registered with Hualalai Resort prior to obtaining a Permit. To minimize violation of Hualalai Resort Work Rules and to reduce Permit Fees/Fines to you, it's suggested that companies have their employees carpool to work sites.

ALL requests must provide proof of Vehicle Insurance & Registration for permit issuance. Supervisors may be contacted to confirm authorization.

HUALALAI NO WORK SCHEDULE & WORK RULES

All of us here at Hualalai Resort take special pride in the appearance of our Resort and are sensitive to the experiences and perceptions of our residents and guests. We ask the same of you as you enter the property to work on various projects.

Quiet weekend work requires a special permit and authorization. Please fill out the Weekend Work Permit Form on the confirmation page after submitting this form.

*Quiet work: No loud equipment or work that causes a nuisance or street congestion in neighborhoods.

​I do hereby agree that myself, my employees and all subcontractors or their agents I employ for the above project shall abide by the rules at all times while on Hualalai Resort property.

I acknowledge having read the HUALALAI RESORT NO WORK SCHEDULES and HUALALAI RESORT WORK RULES. I further acknowledge having notified all members working on my jobsite that any behavior resulting in a negative guest experience will not be tolerated and that immediate action will be taken upon notification of any unacceptable situations.
